11/11/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a content mood evident by her facial expressions such as a smile on the face and engaging in general conversation with QMHPA. Intervention: QMHPA inquired about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA utilized modeled behaviors to assist Ms. Broadnax in remembering and accepting the facts of previous drama, reducing denial and increasing insights into its effects, reducing maladaptive emotional and behavioral responses to traumatic related stimuli, and reducing self-blame. QMHPA assisted Ms. Broadnax in identifying, challenging, and replacing biased, fearful self-talk with reality-based, positive self-talk. QMHPA explored Ms. Broadnaxâs schematic and self-talk that mediated her fears. QMHPA assisted Ms. Broadnax generating thoughts that corrected for the negative biases and built confidence. QMHPA utilized role reversal to assist in demonstrating and practicing with Ms. Broadnax the frequency of speaking up with confidence in social and stress related situations. QMHPA then moved to engage Ms. Broadnax in general conversation in reference to increasing her sense of satisfaction as it relates to her life. QMHPA assisted Ms. Broadnax in developing a plan to include activities that would increase her satisfaction, fulfill her values, and improve the quality of her life. QMHPA reviewed with Ms. Broadnax her strong attempts to modify her life to include previous self-satisfying activities. Response: Ms. Broadnax appeared to be receptive to QMHPAâs services and interventions that were provided to her during the session. Ms. Broadnax verbally shared her thoughts and feelings about getting out of the home and being surrounded by people that are trying to help her become a better person as opposed to what she has been dealing with in the past. Ms. Broadnax stated to QMHPA she is trying to better herself with the help of QMHPA and her god sister. She verbalized she continues to allow her mother and father to make her depressed and she needs to learn not to listen to what they have to say because they are not for her, she feels as though they are against her. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
